Why insurance for a NIE isn't the same as for a non-lucrative residence or a student visa

Every immigration procedure asks you to prove healthcare cover. But how it's reviewed, what they demand and how much weight it carries within your file varies a lot depending on the specific procedure.

There are procedures where private healthcare cover is normally required (such as the NIE for residence, or the non-lucrative residence). There are others where it depends on your situation (such as family reunification or the EU citizen registration). And there are cases where the applicant's age completely changes the viability of the insurance (seniors, retirees, dependent parents).

What they all have in common when insurance is required: it must be a private health policy with no copays, no waiting periods, with hospitalisation and with a certificate that states all of this clearly. But the prior review, the applicant's profile and the documentary details are different in each case.

If you're coming to live in Spain without working (retirement, financial independence), the insurance needs a special review by age. Particularly important if you're over 65.

Typical profile: European, British and North American retirees aged 60-80+. Couples are common.

Student visa

If you're coming to Spain to study, the insurance must have no copays, no waiting periods, no excess and repatriation as per the policy terms. It's worth checking the duration required by your programme and consulate. In many cases annual cover, or cover for the whole stay, is requested.

Family reunification

If you're bringing your spouse, children or parents, the first thing is to know whether you actually need private insurance. In many cases the sponsor's Social Security already covers the relative.

Typical profile: sponsors working in Spain who bring family over. Parents aged 65+ almost always need a policy.

EU citizens & family

If you're European or a family member of one, you may not need private insurance. It depends on whether the EHIC, the S1 form or Social Security already cover you. The guide helps you find out.

What all insurance policies for Spanish immigration have in common

When private health insurance is required (not in every procedure), the requirements coincide on these points:

  • No copays: this is the requirement that causes the most problems. If the certificate doesn't state it explicitly, they may request a correction.
  • No waiting periods: full cover from day one. Waiting times for hospitalisation are not accepted.
  • Cover equivalent to the Spanish public system: emergencies, specialists, diagnostic tests, hospitalisation and surgery.
  • Minimum 12-month validity: contracted in full annual terms, renewable.
  • Official certificate: a document explicitly mentioning no copays, no waiting periods and the main coverages.
  • Can be taken out up to 3 months in advance: with your passport, without needing a Spanish bank account.

What changes between procedures is how viability is reviewed (especially by age), whether a policy is genuinely needed or there's already existing cover, and the documentary details of each file.

Frequently asked questions about health insurance for Spanish procedures

Do all immigration procedures need health insurance?

No. It depends on the type of procedure. For NIE, non-lucrative residence and student visa, yes. For family reunification and EU citizens, it depends on the specific situation.

Does the same insurance work for every procedure?

When insurance is required, the base requirements coincide (no copays, no waiting periods, hospitalisation). But the details of age viability, documentation and prior review change depending on the procedure.

Can I take it out from outside Spain?

Yes, in every case. It can be taken out up to 3 months before travelling, with your passport, without needing a Spanish bank account.

How long does the insurance last?

12 months minimum, in renewable annual terms. It can't be adjusted to 5 or 6 months, even if your stay is shorter.

What if I'm over 65 or 75?
